What the record contains
Determination
A plain-language finding: what was requested, what was provided, what remains outstanding, and whose fault that is — client, firm, or neither. Generated from data, not written by hand.
Full timeline
Every event in chronological order: case opened, items requested, emails sent, opened, clicked, documents uploaded, items accepted or rejected, no-responses logged. Each entry timestamped.
Outstanding items
A clear table of what was requested, what was received, and what is still missing — with fault attribution on each line.
Integrity proof
The SHA-256 head hash of the event chain at the time the record was generated. A reviewer can independently verify the chain has not been altered.
Three possible determinations
The record computes a plain-language determination from the data — no interpretation required.
Fully cooperative
All requested items accepted. Case worked to completion.
Partially cooperative
Some items accepted, others still outstanding — attributed to client.
Non-cooperative
Client failed to provide required documents after repeated attempts.
Why it holds up
A Word document or exported email thread is easy to question. A hash-verified event chain is not.
Generated from data, not authored
The record is computed from the event log. There is no template you fill in, no narrative you write. The conclusions follow from the data.
Hash-chained at the database level
Each event carries the SHA-256 hash of the previous event's canonical representation. UPDATE and DELETE are blocked by database triggers — not application logic.
Verifiable by a third party
The head hash in the record can be verified independently. If any event was altered, the chain breaks and the record says so explicitly.
Shareable without exposing client data
A tokenized read-only link lets you share the record with a refund bank or adjudicator. The link expires. No login required on their end.
Export and share
The record is available on demand for any case at any time. Export it as a PDF or share a tokenized read-only link. The link expires, requires no login from the recipient, and shows the integrity hash so they can verify it independently.
Every export is itself logged as an event — so the record includes a note of when it was generated and reviewed.
